At present I am trying to reduce radiation exposure to salivary glands through X-ray examination by switching to an ultrasound technique that uses microbubbles to image the salivary glands.

  • Head & Neck Cancer

Mark has a long history in the practice of head and neck surgery. Since 1991 he has gathered a database and a pathology archive with over 450 consecutive patient records and specimens. The research at UCLH continues with an active program that includes the application of PET in head and neck. This includes 3D computer augmented reality that allows tumour images to be projected onto the patient in real time.

  • Salivary Tumours

His work has revolutionised the current practice of parotid surgery, moving away from a major to a minor procedure. The research continues to develop minimally invasive surgery for benign salivary tumours. A multi institutional randomised study is planned.

  • Salivary Stones

Professor McGurk has completely changed management of salivary stones. Current treatment policy is to remove the salivary gland. In a series of 500 cases of salivary stones, 86% of calculi have been successfully extracted and only 4% of glands removed. Approximately 3,000 patients each year in the UK who would normally have their glands removed can now be treated successfully by minimally invasive means.

  • Day Case Oral Surgery

This involves mainly removal of impacted third molar teeth and is one of the commonest procedures undertaken in the NHS. The work has looked at efficient provision of this service and has investigated factors that predict for difficult cases and complication of surgery.

  • Biomaterials

An active programme of biomaterial research with a team of scientists focused on a bone replacement material is in process. This involves developing new scaffolds and augmenting their use with stem cell extracts. We provide expertise with the animal model. We are pioneering the use of 3D computer models in conjunction with rapid prototyping techniques to produce parons scaffolds tailor-made to replace bone units in the face. This technique provides immediate facial reconstruction with a bioactive material to a very high accuracy.

   Personal Statement

Clinically and personally my work is highly rewarding. I gain a great deal of personal satisfaction from helping people to recover and from trying to ensure that they have as positive an experience as possible.

 

My career has been divided between research, teaching and consultancy. My life's work has been to simplify surgery and make it smaller and easier to bear, yet I have long been aware of the slow rate of change within the profession and of the length of time it can take for life-saving improvements to become part of standard practice. I have sought to combat this by focusing my work within teaching hospitals in order to use their resources and to maintain close contact with new generations of practitioners and consultants.

 

In my teaching, travelling and lecturing I interact and receive feedback from some of the most promising medical students in the world. I value this constant interaction very highly and I feel that it is a very important duty to pass on my clinical experience and research. 

 

Professionally I am grateful that much of my work has gained recognition both in the UK and internationally and that to a large degree I have progressed towards my main career goal of simplifying surgery and making it easier to bear.

 

I am very aware that economic inequalities deprive many people of the top-level care that is available to a relatively small number of people. I spend time in Ethiopia each year as part of a medical charity team with  Project Harar and recently started the Head and Neck Cancer Foundation

  2. Improvements in the surgical treatment of head and neck cancer using minimally invasive techniques and the setting up of a national training program at UCLH in 2017 to introduce SNB (sentinel node biopsy) in UK for the detection of cancer cells, thus avoiding exploratory neck surgery.

  3. Improvements in the treatment of benign parotid tumours and salivary gland disorders, it is now no longer necessary to routinely remove salivary glands for benign disease.

  4. Initiated a UCL controversy series of symposia to promote a more conservative surgical approach to  disorders in the head and neck.

  5. Setting up a surgical master class program to disseminate our research and improve minimally invasive surgical techniques among interns and practicing surgeons. This was started at Guys Hospital and is now transferred to King Edward VII's. (Jan 2017) These classes are also organised in Paris and Erlangen for maxillofacial and ENT consultants.

  6. Developing a new way to treat odontogenic tumours (ameloblastoma) which preserves the mandible and maintains normal facial appearance.

  8. Developed an approach to planned operations by rehearsing with printed models of the patients. This approach became part of our research and is now  integrated into an increasing number of modern practices.

     

    8.  Running a European trial in sentinel node biopsy (SNB) which has led to a 70% reduction in the need for neck dissection for early mouth cancer. This work has been recognised by NICE who have advised that the test should be available throughout the UK.
